Responsibilities:
- Develop and oversee personalized care plans.
- Coordinate care with healthcare providers and ensure quality standards.

Qualifications:
- Bachelor’s degree in Nursing, Social Work, or a related field preferred.
- Experience in case management or care coordination is a plus.
- Strong organizational and interpersonal skills.
