Responsibilities:

• Assess client and family needs, providing counseling and support.
• Help clients access community resources and healthcare services.
• Collaborate with the healthcare team to create comprehensive care plans.
• Provide crisis intervention and support during transitional care.

Qualifications:

• Bachelor’s or Master’s degree in Social Work.
• Current social work license in Georgia (LMSW or LCSW preferred).
• Experience in a healthcare or community setting is a plus.
• Strong communication and organizational skills.
